Q: In what ways do you feel the clinical trial industry is making tradeoffs that can be improved upon with the application of AI/ML tools?

In today’s clinical trial landscape, study teams must often make tradeoffs between speed, quality, and cost. For example, a sponsor may rush protocol submission to meet corporate milestones—but that can introduce restrictive eligibility criteria or burdensome visit schedules, triggering downstream amendments. Other times, scientific ambition may overshadow feasibility, leading to site resistance or patient enrollment challenges. These tradeoffs are driven by incomplete evidence and fragmented decision inputs. AI and machine learning offer a way to break this cycle—providing predictive insights that allow for more balanced decisions across study design, operational feasibility, and patient impact.

One of the most persistent tradeoffs in clinical trials involves inclusivity. Sponsors often start with good intentions to support broader populations—linguistically, geographically, and demographically—but when timelines and budgets tighten, those aspirations are among the first things cut. The decision is rarely based on whether something is possible. It’s about whether it’s feasible under current timelines and operational pressure. The result is that studies often over-index on English-speaking or Western European participants because localization into other languages or regions is seen as too time-consuming.

These tradeoffs don’t stem from a lack of access to qualified linguists or resources. We’ve always had the ability to translate and support diverse populations. What we lack is efficiency. Study teams are forced to make early-stage decisions based on how long something might take, not based on what’s right for patient access or trial quality. And the impact is tangible—underserved populations continue to be excluded from research, and sponsors miss opportunities to generate more representative data.

AI doesn’t suddenly make these populations reachable, but it can make including them far less burdensome. By eliminating repetitive, manual steps in localization and document preparation, AI helps study teams reclaim time. That time creates space to include more languages, more geographies, and ultimately more patients. Instead of making tradeoffs, sponsors can start making progress.

Q: How can AI technologies and solutions be leveraged to avoid making these tradeoffs?

Can you provide a few specific examples of how you have seen AI/ML being used to increase capacity and efficiency? (e.g. Study Start-ups, Translation, Localization, etc.)

AI can strengthen both efficiency and decision quality throughout the entire study design, planning, and startup process:

  • Predictive modeling can flag protocol elements likely to cause amendments, giving sponsors the chance to adjust early.
  • AI can act as a translation and localization agent, adapting study materials for different patients and sites across audiences and geographies—while drawing on a shared dataset to preserve accuracy and compliance.
  • Natural language models can classify, extract, and reconcile study documents more quickly and accurately than manual review, freeing teams from administrative work.
  • Machine learning can improve enrollment forecasting, helping teams plan scenarios and align expectations.

In each case, AI not only automates tasks but improves the reliability of outputs, enabling staff to focus on higher-value, strategic decisions.

AI is especially impactful in localization, where the pain isn’t in the translation itself, it’s in the process. In eCOA and other patient-facing systems, only a small portion of the timeline is spent on creating the target language wording. The rest is consumed by repetitive, manual tasks: moving content between “paper” documents and platforms, comparing screen texts across languages, validating layout, and reconciling version control. These are not high-skill tasks, but they are high-volume and time-consuming.

At YPrime, we’ve implemented AI-powered tools to tackle exactly this kind of work. One example is our eCOA migration tool, which automates the transfer of validated content from licensed questionnaires into digital platforms. It used to take weeks to complete this work across a full study; now it takes hours, and with higher accuracy. Human linguists are still essential, but now they’re spending their time reviewing high-quality first drafts instead of correcting formatting errors.

In one recent study, a large sponsor needed to add India mid-trial, an expansion that typically would have been ruled out due to the lead time for Indic language localization. By using our AI-enabled tools, we were able to skip the slowest steps, deliver first-pass screen quality that met approval standards, and get those languages live in the same timeline as French or Spanish. It’s a perfect example of how AI doesn’t replace expertise, it clears the way for it.

Q: To that end, what other areas of clinical research operations do you believe could benefit most from leveraging AI/ML? How do you see these improvements being applied more broadly?

AI has enormous potential to shift trial operations from reactive adjustments to proactive planning and execution. Several areas could see significant gains:

  • Site feasibility and selection can be informed by patterns in performance, allowing sponsors to better align site strategy with study needs.
  • Generative models can be used to customize patient outreach, informed consent, and educational materials at scale while maintaining regulatory alignment.
  • Machine learning can flag operational risks, such as delays or data inconsistencies, before they escalate into larger issues.

When applied broadly, these approaches have the potential to make studies more efficient, predictable, and less burdensome for participants and sites.

There’s a huge opportunity to apply AI to operational knowledge management across trials. Clinical research is a high-repeatability environment with similar protocols, similar content, and often the same study teams. And yet, we regularly see organizations start from scratch. AI could help identify overlaps in previous studies, pre-validate content, and recommend configuration based on historical inputs. It’s about turning “what we’ve done before” into a strategic advantage, not a forgotten archive.

One specific area is translation workflows and copyright approvals. AI models can be trained on historical feedback from copyright holders to preemptively flag issues in new studies. That alone could shave weeks off timelines. Similarly, distinguishing high-risk from low-risk content (such as questionnaire items vs. UI labels) can help streamline review processes. Low-risk content can move through faster, freeing human attention for areas that matter most.

We’re also seeing early signals of AI supporting real-time quality reviews—scanning outputs against known standards, highlighting potential inconsistencies, and learning from recurring audit findings. These models don’t need to replace the quality assurance process, but they can frontload it. As more organizations adopt AI, the industry will shift from reactive correction to proactive prevention. That’s where the true operational ROI will come from.

Q: Laura, how can emerging AI technologies and unified data ecosystems serve as a force multiplier within the clinical research industry? What role does centralized data play in that?

The impact of AI depends entirely on the quality—and connectedness—of the data behind it. In many organizations, trial operations data remains siloed across paper-based protocols, disparate systems, and site records. This fragmentation limits the ability of AI models to learn from past outcomes and provide reliable predictions.

When data is centralized, harmonized, and connected across trial design and operations (think: datasets that integrate protocols, amendment history, operational metrics, and site performance), AI tools can surface feasibility insights, provide more accurate benchmarks, and forecast risks that were previously undetectable. The result is not merely automation but better decision support across the trial lifecycle—transforming AI into a true force multiplier for efficiency, predictability, and quality in clinical research.

Q: Jonathan, on the podcast, you challenged the presumption that AI allows us to execute tasks that we otherwise would not be able execute without it. Can you expand on this?

When it comes to language knowledge, AI doesn’t just unlock new capabilities, it unlocks capacity. In clinical trials, we’ve always had access to multilingual translators. We’ve always had the ability to reach underrepresented patients. What we haven’t had is a scalable way to do that efficiently, especially under compressed timelines. AI isn’t making localization possible—it’s making it practical.

At YPrime, my approach is simple: if you’re looking for a deep dive into the risks of using AI, there are plenty of other places to go. The conversation needs to move on from this. We use AI with intention, with oversight, and always with a human in the loop. It should not be replacing decisions; it should be replacing grunt work. And in operations, that distinction is everything.

What we need to stop doing is treating AI like a magic bullet or, conversely, a minefield. It’s neither, it’s a tool that’s already delivering results. The challenge isn’t whether to use AI, it’s how to use it effectively, safely, and at scale. In my world, that means using proven tools to eliminate unnecessary steps so humans can focus on the value-added work. That’s not about replacing people, it’s about elevating them.
